INDICATION

Zolki 600mg Tab is an oxazolidinone antibiotic indicated for infections caused by susceptible gram-positive organisms, including vancomycin-resistant Enterococcus faecium (VRE), nosocomial and community-acquired pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA), and complicated skin and soft tissue infections.

MECHANISM OF ACTION

Linezolid is a synthetic bacteriostatic antibiotic that inhibits bacterial protein synthesis by binding to the 23S ribosomal RNA of the 50S subunit in a site distinct from all other antibiotics, preventing the formation of the 70S initiation complex required to begin translation. Its unique binding site means there is no cross-resistance with other antibiotic classes, making linezolid effective against multidrug-resistant gram-positive organisms including MRSA and VRE.

DOSAGE AND ADMINISTRATION

•      Adults: 600mg twice daily every 12 hours for 10 to 28 days depending on indication

•      May be taken with or without food

•      Weekly CBC monitoring is mandatory for courses exceeding 2 weeks

•      Always used under hospital or specialist supervision for serious infections

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea

•      Headache

•      Taste disturbances or tongue discoloration

•      Elevated liver enzymes

Less common:

•      Myelosuppression: anemia, thrombocytopenia, neutropenia (monitor CBC weekly)

•      Peripheral and optic neuropathy with prolonged therapy

•      Serotonin syndrome when combined with serotonergic drugs

•      Lactic acidosis (rare but serious)

NOT RECOMMENDED IN

•      Known hypersensitivity to linezolid or oxazolidinones

•      Concurrent MAO inhibitors

•      Uncontrolled hypertension or phaeochromocytoma without monitoring

•      Pregnancy and breastfeeding unless benefit clearly outweighs risk

DRUG INTERACTIONS

Linezolid is a reversible non-selective MAO inhibitor; concurrent SSRIs, SNRIs, TCAs, triptans, and tramadol risk serotonin syndrome. Adrenergic drugs cause exaggerated pressor responses. Tyramine-rich foods must be avoided. Rifampicin reduces linezolid levels.
