INDICATION

Swissmether 40/240mg Tab is an artemisinin-based combination antimalarial indicated for the treatment of uncomplicated acute malaria caused by Plasmodium falciparum, including areas with reported chloroquine or multidrug resistance, in adults and children weighing 35kg or more.

MECHANISM OF ACTION

Artemether is a semi-synthetic derivative of artemisinin that is rapidly converted in the body to its active metabolite dihydroartemisinin (DHA). DHA generates reactive oxygen species and free radicals through interaction with haem iron within the parasite food vacuole, causing oxidative damage to parasite proteins and membranes and producing rapid parasite killing within the first 48 hours. Lumefantrine is a long-acting antimalarial that inhibits the formation of hemozoin from toxic haem released during hemoglobin digestion, causing toxic haem accumulation within the parasite. The combination ensures rapid initial parasite clearance by artemether followed by elimination of residual parasites by lumefantrine, reducing the risk of recrudescence and resistance development.

DOSAGE AND ADMINISTRATION

•      Adults and children above 35kg: 4 tablets (artemether 160mg + lumefantrine 960mg) twice daily for 3 days (6 doses total)

•      Take with food or a fatty drink to enhance lumefantrine absorption

•      Doses should be taken approximately 8 hours apart on day 1, then twice daily on days 2 and 3

•      Always follow the complete 3-day course as prescribed; never stop early even if symptoms resolve

SIDE EFFECTS

Common:

•      Headache and dizziness

•      Nausea, vomiting, and abdominal pain

•      Fatigue and asthenia

•      Arthralgia and myalgia

Less common:

•      QT prolongation (monitor ECG in patients with cardiac risk factors)

•      Sleep disturbances and insomnia

•      Palpitations

•      Elevated liver enzymes

NOT RECOMMENDED IN

•      Known hypersensitivity to artemether, lumefantrine, or any excipient

•      Severe or complicated malaria requiring parenteral therapy

•      Concurrent use with other QT-prolonging drugs or strong CYP3A4 inhibitors

•      First trimester of pregnancy unless no other effective treatment is available

DRUG INTERACTIONS

Drugs that prolong the QT interval including antiarrhythmics, antipsychotics, macrolide antibiotics, and fluoroquinolones increase the risk of cardiac arrhythmia; avoid concurrent use. Strong CYP3A4 inhibitors such as ketoconazole and ritonavir increase lumefantrine exposure; use with caution. CYP3A4 inducers including rifampicin, carbamazepine, and phenytoin reduce lumefantrine plasma levels, risking treatment failure; avoid concurrent use. Lumefantrine is a CYP2D6 inhibitor and may increase levels of drugs metabolized by this enzyme including metoprolol and some antidepressants.
