INDICATION

Swisat 100mg Tab is an atypical antipsychotic indicated for the treatment of schizophrenia, manic episodes in bipolar disorder, and as adjunctive therapy for major depressive disorder. The 100mg dose is commonly used as a titration step toward therapeutic doses in schizophrenia and bipolar disorder management.

MECHANISM OF ACTION

Quetiapine acts as an antagonist at dopamine D1 and D2 receptors, serotonin 5-HT1A (partial agonist) and 5-HT2A receptors, histamine H1 receptors, and alpha-1 and alpha-2 adrenergic receptors. The combined receptor profile provides antipsychotic, mood-stabilizing, and anxiolytic effects. Its active metabolite Norquetiapine is a norepinephrine reuptake inhibitor, contributing to antidepressant efficacy. H1 receptor antagonism contributes to sedation, which can be therapeutically useful in managing agitation and improving sleep.

DOSAGE AND ADMINISTRATION

•      Schizophrenia: titrate from 25mg to 50mg on day 1 and 2; increase to 100mg on day 3 and 4; target 300 to 600mg daily

•      Bipolar mania: 100mg on day 1, increasing by 100mg/day to 400 to 800mg

•      Take with or without food

•      Always follow the titration schedule prescribed by your psychiatrist; never stop abruptly

SIDE EFFECTS

Common:

•      Somnolence and sedation

•      Dry mouth

•      Dizziness and orthostatic hypotension

•      Weight gain

Less common:

•      Metabolic syndrome: hyperglycemia, dyslipidemia with long-term use

•      QT prolongation

•      Extrapyramidal symptoms (less common than typical antipsychotics)

•      Tardive dyskinesia with prolonged use

NOT RECOMMENDED IN

•      Known hypersensitivity to quetiapine

•      Concurrent strong CYP3A4 inhibitors without dose adjustment

•      Elderly patients with dementia-related psychosis (increased mortality risk)

DRUG INTERACTIONS

Strong CYP3A4 inhibitors markedly increase quetiapine levels; reduce dose significantly. CYP3A4 inducers dramatically reduce quetiapine efficacy. CNS depressants and alcohol add to sedation. Antihypertensives add to hypotension. QT-prolonging drugs increase arrhythmia risk.
