INDICATION

Prozac 20mg Cap is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder (MDD), obsessive-compulsive disorder (OCD), bulimia nervosa, panic disorder with or without agoraphobia, and as part of combination therapy for treatment-resistant depression with olanzapine.

MECHANISM OF ACTION

Fluoxetine selectively inhibits the reuptake of serotonin (5-hydroxytryptamine) into presynaptic neurons by blocking the serotonin transporter (SERT), increasing serotonin availability in the synaptic cleft. With minimal affinity for muscarinic, histaminic, or adrenergic receptors, fluoxetine has a cleaner side-effect profile than older antidepressants. Therapeutic effects typically emerge after 2 to 4 weeks as serotonergic neurotransmission normalizes and downstream neuroplastic adaptations occur. Its active metabolite norfluoxetine has a half-life of 4 to 16 days, providing extended pharmacological activity.

DOSAGE AND ADMINISTRATION

•      Depression: 20mg once daily in the morning; may increase to 40 to 60mg after several weeks if inadequate response

•      OCD and bulimia: 40 to 60mg per day; start at 20mg and titrate

•      Panic disorder: 10mg per day initially, increasing to 20mg after 1 week; usual dose 20 to 60mg

•      Always follow the dosage and titration schedule prescribed by your psychiatrist; may take 4 to 6 weeks for full effect

SIDE EFFECTS

Common:

•      Nausea (most common, usually transient)

•      Insomnia and sleep disturbances

•      Headache and dizziness

•      Dry mouth, sweating

Less common:

•      Sexual dysfunction (reduced libido, delayed orgasm, erectile dysfunction)

•      Weight loss initially, weight gain with prolonged use

•      Serotonin syndrome when combined with serotonergic drugs (rare but serious)

DRUG INTERACTIONS

MAO inhibitors are contraindicated; a 14-day washout is required before starting fluoxetine and a 5-week washout after stopping fluoxetine before starting an MAOI (due to long half-life). Fluoxetine is a potent CYP2D6 inhibitor, significantly increasing levels of tricyclic antidepressants, antipsychotics including haloperidol, and some beta-blockers. Tamoxifen efficacy is reduced by fluoxetine through CYP2D6 inhibition of its activation. Lithium and tryptophan increase serotonin syndrome risk.
