INDICATION
Progrel 75mg Tablets contain clopidogrel, an antiplatelet agent indicated for the reduction of atherothrombotic events in patients with a history of recent myocardial infarction, recent stroke, or established peripheral arterial disease. It is also used in combination with aspirin for the prevention of atherothrombotic events in patients with acute coronary syndrome (unstable angina or non-ST elevation myocardial infarction) and in patients undergoing percutaneous coronary intervention (stent placement).
MECHANISM OF ACTION
Clopidogrel is a thienopyridine prodrug that is metabolized in the liver by CYP2C19 to its active metabolite. The active metabolite irreversibly binds to the P2Y12 ADP receptor on the platelet surface, preventing ADP-mediated activation of the glycoprotein GPIIb/IIIa complex, which is the final common pathway for platelet aggregation. By blocking this pathway, clopidogrel reduces platelet stickiness and prevents the formation of arterial blood clots.
DOSAGE AND ADMINISTRATION
● Adults: 75mg once daily with or without food
● For acute coronary syndrome: a loading dose of 300mg followed by 75mg daily in combination with aspirin
● Take at the same time each day
● Swallow tablets whole; do not crush or chew
● Do not stop taking without consulting your cardiologist, as abrupt discontinuation may increase the risk of cardiovascular events
● Continue for the duration prescribed, which may be lifelong in some patients
SIDE EFFECTS
Common:
● Bleeding, which is the most significant adverse effect (bruising, nosebleeds, GI bleeding, prolonged bleeding from cuts)
● Diarrhea and abdominal pain
● Indigestion and heartburn
Less common:
● Thrombotic thrombocytopenic purpura (TTP), a rare but serious blood disorder
● Gastrointestinal hemorrhage or intracranial bleeding
● Rash and pruritus
● Hepatitis and elevated liver enzymes (rare)
NOT RECOMMENDED IN
● Known hypersensitivity to clopidogrel or any component
● Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
● Severe hepatic impairment with coagulation disorders
● Within 7 days of planned surgery unless the cardiovascular benefit outweighs bleeding risk
DRUG INTERACTIONS
Proton pump inhibitors, especially omeprazole and esomeprazole, may reduce clopidogrel activation via CYP2C19 inhibition. NSAIDs and aspirin increase bleeding risk. Warfarin combined with clopidogrel significantly increases hemorrhagic risk. CYP2C19 inhibitors (fluconazole, fluoxetine, ticlopidine) may reduce clopidogrel efficacy.
