INDICATION
Prelox Plus Syrup is a third-generation cephalosporin antibiotic suspension indicated for the treatment of mild to moderate bacterial infections in pediatric patients. It is prescribed for upper respiratory tract infections including pharyngitis, tonsillitis, and acute otitis media, lower respiratory tract infections including community-acquired pneumonia and acute bronchitis, and uncomplicated urinary tract infections. Cefpodoxime proxetil is an orally absorbed prodrug that is hydrolyzed to its active form in the gastrointestinal tract.
MECHANISM OF ACTION
Cefpodoxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) and disrupting the final transpeptidation step of peptidoglycan cross-linking. This weakens the bacterial cell wall, leading to osmotic lysis and cell death. Cefpodoxime has a broad spectrum of activity against gram-positive organisms including Streptococcus pneumoniae and S. pyogenes, and gram-negative organisms including Haemophilus influenzae, Moraxella catarrhalis, and E. coli.
DOSAGE AND ADMINISTRATION
● Children 2 months to 12 years: 5mg/kg/dose twice daily (maximum 200mg per dose)
● For otitis media: 5mg/kg twice daily for 5 to 10 days
● For pharyngitis and tonsillitis: 5mg/kg twice daily for 5 to 10 days
● Administer with food to enhance absorption
● Shake the bottle well before measuring each dose
● Complete the full course even if symptoms improve early
SIDE EFFECTS
Common:
● Diarrhea and loose stools
● Nausea and vomiting
● Diaper rash in infants
● Abdominal discomfort
Less common:
● Allergic reactions including rash, urticaria, and rarely anaphylaxis
● Pseudomembranous colitis caused by Clostridium difficile
● Elevated liver enzymes
● Headache and dizziness
NOT RECOMMENDED IN
● Known hypersensitivity to cefpodoxime, any cephalosporin, or penicillin with confirmed anaphylactic history
● Severe renal impairment without dose adjustment
● History of antibiotic-associated colitis
DRUG INTERACTIONS
Antacids and H2-receptor antagonists reduce cefpodoxime absorption and should be taken at least 2 hours apart. Probenecid increases cefpodoxime plasma levels by reducing renal clearance. No clinically significant interaction with oral contraceptives.
STORAGE INSTRUCTIONS
Store dry granules below 25 degrees Celsius. After reconstitution, refrigerate between 2 to 8 degrees Celsius and use within 14 days. Do not freeze. Keep out of reach of children.
