INDICATION
Plivil Injection 2ML contains pheniramine maleate, a first-generation antihistamine (H1-receptor antagonist) administered by intramuscular or intravenous route. It is indicated for the treatment of acute allergic reactions including urticaria, angioedema, allergic rhinitis, drug-induced allergic reactions, transfusion reactions, and as an adjunct in the management of anaphylaxis alongside epinephrine. The injectable form provides rapid onset of antihistaminic action when oral administration is not feasible or when immediate symptom control is needed.
MECHANISM OF ACTION
Pheniramine maleate competitively blocks histamine H1 receptors on effector cells in the respiratory tract, gastrointestinal tract, blood vessels, and skin. By preventing histamine from binding to these receptors, it reduces the allergic response including vasodilation, increased capillary permeability, bronchoconstriction, smooth muscle contraction, and pruritus. As a first-generation antihistamine, pheniramine also crosses the blood-brain barrier, which accounts for its sedative properties. The injectable route provides a rapid onset of action within 15 to 30 minutes.
DOSAGE AND ADMINISTRATION
● Adults: 22.75 to 45.5mg (1 to 2ml) by slow intravenous or intramuscular injection
● May be repeated every 4 to 6 hours as needed based on clinical response
● For anaphylaxis: administer as an adjunct after epinephrine, not as a first-line treatment
● Intravenous administration should be given slowly over at least 1 minute to avoid hypotension
● Must be administered by trained healthcare personnel only
● Monitor the patient for drowsiness, hypotension, and respiratory depression after administration
SIDE EFFECTS
Common:
● Drowsiness and sedation, which are the most frequently reported effects
● Dry mouth and throat
● Dizziness and lightheadedness
● Injection site pain
Less common:
● Hypotension, particularly with rapid intravenous administration
● Urinary retention, especially in elderly males with prostatic hypertrophy
● Blurred vision and difficulty with accommodation
● Paradoxical excitation, restlessness, and irritability, particularly in children
NOT RECOMMENDED IN
● Known hypersensitivity to pheniramine maleate or any antihistamine
● Neonates and premature infants
● Patients with narrow-angle glaucoma
● Patients with urinary retention or symptomatic prostatic hypertrophy
● Concurrent use with monoamine oxidase inhibitors (MAOIs), as they may prolong and intensify the anticholinergic effects of antihistamines
DRUG INTERACTIONS
CNS depressants including alcohol, benzodiazepines, opioids, and barbiturates enhance the sedative effect of pheniramine and should be used with caution. MAOIs prolong and intensify the anticholinergic effects. Pheniramine may mask the ototoxic symptoms of aminoglycoside antibiotics. Concurrent use with other anticholinergic drugs may produce additive effects including dry mouth, urinary retention, and constipation.
