INDICATION

Petra 40mg Caps is a higher-strength PPI indicated for severe or refractory GERD, erosive esophagitis, Zollinger-Ellison syndrome, H. pylori eradication, and prevention of recurrence of peptic ulcers in patients on NSAIDs. The 40mg dose is prescribed when the 20mg dose is insufficient for adequate acid suppression.

MECHANISM OF ACTION

Esomeprazole at 40mg achieves a higher degree of H+/K+-ATPase inhibition and more sustained acid suppression than the 20mg dose. It provides a mean intragastric pH above 4 for longer periods throughout the day, which is critical for healing erosive esophagitis and managing hypersecretory states. The extended bioavailability profile of esomeprazole compared to omeprazole ensures consistent acid suppression regardless of CYP2C19 metabolizer status in most patients.

DOSAGE AND ADMINISTRATION

•      Severe GERD or erosive esophagitis: 40mg once daily for 4 to 8 weeks

•      Zollinger-Ellison syndrome: 40mg twice daily; titrated based on acid output

•      H. pylori eradication: 40mg twice daily with appropriate antibiotics for 7 to 14 days

•      Take 30 to 60 minutes before meals; always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Headache

•      Nausea, vomiting, diarrhoea

•      Abdominal pain or flatulence

•      Dry mouth

Less common:

•      Hypomagnesaemia causing muscle cramps, arrhythmias, and seizures with prolonged use

•      Clostridium difficile colitis

•      Vitamin B12 deficiency

•      Bone fracture risk with very long-term high-dose PPI use

•      Known hypersensitivity to esomeprazole or other PPIs

•      Concurrent use with atazanavir or nelfinavir

DRUG INTERACTIONS

Same interaction profile as Petra 20mg. Esomeprazole inhibits CYP2C19 and reduces the bioactivation of clopidogrel less than omeprazole. Reduces absorption of ketoconazole, itraconazole, and pH-dependent drugs. Monitor digoxin and methotrexate levels. Avoid with nelfinavir and atazanavir.
