INDICATION
Oxa-One Injection is an oxazolidinone antibiotic indicated for the treatment of serious infections caused by susceptible gram-positive organisms. It is prescribed for vancomycin-resistant Enterococcus faecium (VRE) bacteremia, nosocomial and community-acquired pneumonia caused by MRSA or penicillin-resistant Streptococcus pneumoniae, and complicated skin and soft tissue infections caused by MRSA. The IV formulation is used when oral administration is not possible or when rapid, reliable drug delivery is needed in critically ill patients.
MECHANISM OF ACTION
Linezolid inhibits bacterial protein synthesis by binding to the 23S rRNA of the 50S ribosomal subunit, preventing the formation of the 70S initiation complex essential for translation. This unique mechanism means cross-resistance with other antibiotic classes does not occur. Linezolid is bacteriostatic against enterococci and staphylococci and bactericidal against most streptococci.
DOSAGE AND ADMINISTRATION
● Adults: 600mg IV infusion every 12 hours over 30 to 120 minutes
● Children: 10mg/kg IV every 8 hours, maximum 600mg per dose
● Duration: 10 to 14 days for skin infections, 14 to 28 days for pneumonia and VRE
● Do not mix with other IV medications in the same line
● Must be administered by trained healthcare personnel
● Monitor complete blood count weekly, especially platelets
SIDE EFFECTS
Common:
● Diarrhea and nausea
● Headache
● Thrombocytopenia, requiring weekly platelet monitoring
● Infusion site reactions
Less common:
● Myelosuppression including anemia, leukopenia, and pancytopenia with therapy beyond 2 weeks
● Peripheral and optic neuropathy with prolonged use
● Lactic acidosis with extended therapy
● Serotonin syndrome with concurrent serotonergic agents
NOT RECOMMENDED IN
● Known hypersensitivity to linezolid or any oxazolidinone
● Concurrent use with MAOIs or within 2 weeks of MAOI discontinuation
● Uncontrolled hypertension or pheochromocytoma
● Concurrent use with serotonergic agents without careful risk assessment

Linezolid is a weak, reversible MAO inhibitor. Serotonergic drugs (SSRIs, SNRIs, triptans, meperidine, tramadol) may cause serotonin syndrome. Limit tyramine-rich foods during treatment. Sympathomimetics may increase blood pressure.
STORAGE INSTRUCTIONS
Store at room temperature between 15 to 30 degrees Celsius. Protect from light and freezing. Use immediately after removing overwrap. Do not use if the solution is cloudy or contains particles. Keep out of reach of children.
