INDICATION

Optopred Drops 5ML is a corticosteroid ophthalmic solution indicated for the treatment of steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe. It is prescribed for allergic conjunctivitis, iritis, cyclitis, anterior uveitis, chemical and thermal burns, and post-operative inflammation following ocular surgery. Prednisolone is one of the most potent topical ophthalmic corticosteroids.

MECHANISM OF ACTION

Prednisolone acetate is a synthetic glucocorticoid that suppresses the inflammatory response by multiple mechanisms. It inhibits phospholipase A2, reducing the production of arachidonic acid metabolites including prostaglandins and leukotrienes. It stabilizes lysosomal membranes, reduces capillary permeability, decreases leukocyte migration and phagocytosis, and suppresses the production of inflammatory cytokines. The 1.0% concentration provides potent anti-inflammatory activity for moderate to severe ocular inflammation.

DOSAGE AND ADMINISTRATION

     For mild inflammation: instill 1 to 2 drops in the affected eye(s) 2 to 4 times daily

     For severe inflammation: instill 1 to 2 drops every hour during waking hours, reducing frequency as inflammation subsides

     Shake the bottle well before each use, as prednisolone is a suspension

     Do not discontinue abruptly; taper gradually to prevent rebound inflammation

     Treatment duration depends on clinical response; reassess regularly

     Do not wear contact lenses during treatment

SIDE EFFECTS

Common:

     Mild burning or stinging upon instillation

     Blurred vision immediately after application

     Increased tearing

Less common:

     Elevated intraocular pressure (steroid-induced glaucoma), especially with prolonged use

     Posterior subcapsular cataract formation with extended therapy

     Secondary ocular infection due to immunosuppression

     Delayed wound healing after corneal surgery or injury

NOT RECOMMENDED IN

     Herpes simplex keratitis (dendritic ulcer) and other viral eye diseases

     Mycobacterial or fungal eye infections

     Known hypersensitivity to prednisolone or any component

     Acute untreated purulent bacterial eye infections without concurrent antibiotic therapy

DRUG INTERACTIONS

Concurrent use with other corticosteroid eye drops may cause cumulative steroid effects and elevated IOP. NSAIDs used concurrently may increase corneal healing complications. Anticholinesterase agents for glaucoma may be less effective with concurrent corticosteroid use.

STORAGE INSTRUCTIONS

Store between 2 to 25 degrees Celsius. Do not freeze. Shake well before use. Protect from light. Discard 4 weeks after opening. Keep out of reach of children.
