INDICATION
Optim Eye Drop 5ML is a non-selective beta-adrenergic receptor blocker indicated for the reduction of elevated intraocular pressure in patients with open-angle glaucoma, aphakic glaucoma, and ocular hypertension. Timolol is one of the most established first-line treatments for glaucoma and may be used alone or as an adjunct with other IOP-lowering agents including prostaglandin analogs, carbonic anhydrase inhibitors, or alpha-agonists.
MECHANISM OF ACTION
Timolol maleate reduces intraocular pressure by decreasing the production of aqueous humor in the ciliary body. It blocks beta-2 adrenergic receptors on the ciliary epithelium, which are involved in the active secretion of aqueous humor. The IOP-lowering effect begins within 30 minutes, peaks at 1 to 2 hours, and lasts approximately 12 to 24 hours, allowing twice-daily dosing for sustained pressure control.
DOSAGE AND ADMINISTRATION
● Instill 1 drop in the affected eye(s) twice daily, morning and evening
● Maintain consistent 12-hour intervals between doses
● Apply nasolacrimal occlusion for 2 minutes after each dose to minimize systemic absorption
● Remove contact lenses before instillation and wait 15 minutes before reinserting
● Do not discontinue abruptly; consult your ophthalmologist before stopping
● Monitor intraocular pressure regularly during treatment
SIDE EFFECTS
Common:
● Mild eye irritation and stinging upon instillation
● Dry eyes
● Blurred vision
● Slight headache
Less common:
● Systemic beta-blockade effects including bradycardia, hypotension, and bronchospasm
● Exacerbation of asthma or COPD
● Depression, fatigue, and sleep disturbances
● Corneal anesthesia with prolonged use
NOT RECOMMENDED IN
● Bronchial asthma or severe COPD
● Sinus bradycardia, second or third-degree heart block, or overt cardiac failure
● Uncompensated heart failure

DRUG INTERACTIONS
Concurrent use with systemic beta-blockers produces additive cardiac effects. Calcium channel blockers may cause AV conduction disturbances. Digitalis glycosides may produce additive bradycardia. CYP2D6 inhibitors may increase systemic timolol levels.
