INDICATION
Optibrex Eye Drop is an aminoglycoside antibiotic ophthalmic solution indicated for the treatment of external infections of the eye and its adnexa caused by susceptible bacteria. It is prescribed for bacterial conjunctivitis, blepharitis, keratitis, and dacryocystitis. Tobramycin has excellent activity against gram-negative bacteria, particularly Pseudomonas aeruginosa, making it a first-line choice for suspected pseudomonal eye infections.
MECHANISM OF ACTION
Tobramycin binds irreversibly to the 30S ribosomal subunit of susceptible bacteria, interfering with the initiation complex between mRNA and the ribosome. This causes misreading of the genetic code and inhibits bacterial protein synthesis, resulting in bactericidal activity. Tobramycin demonstrates potent activity against gram-negative organisms including Pseudomonas aeruginosa, E. coli, Klebsiella, Proteus, and Enterobacter, and also covers some gram-positive organisms including Staphylococcus aureus.
DOSAGE AND ADMINISTRATION
● For mild to moderate infections: instill 1 to 2 drops in the affected eye(s) every 4 hours
● For severe infections: instill 2 drops every hour until improvement, then reduce frequency
● Treatment duration: 7 to 10 days or as directed by the ophthalmologist
● Wash hands before and after application
● Do not touch the dropper tip to the eye or any surface
● If using other eye drops concurrently, space at least 5 minutes apart
SIDE EFFECTS
Common:
● Mild burning or stinging upon instillation
● Eye redness and irritation
● Tearing
● Eyelid itching
Less common:
● Allergic conjunctivitis or eyelid dermatitis
● Corneal toxicity with prolonged use
● Overgrowth of non-susceptible organisms including fungi
● Periocular edema
NOT RECOMMENDED IN
● Known hypersensitivity to tobramycin or any aminoglycoside antibiotic
● Viral or fungal eye infections
● Extended use beyond 10 days without review
DRUG INTERACTIONS
Avoid concurrent topical use with other aminoglycosides. Systemic aminoglycoside use alongside topical tobramycin may increase ototoxicity and nephrotoxicity risk, though ocular absorption is minimal.
