INDICATION

Optachlor Eye Drop is a broad-spectrum bacteriostatic antibiotic indicated for the treatment of superficial bacterial infections of the eye including bacterial conjunctivitis, blepharitis, and keratitis caused by susceptible organisms. Chloramphenicol has one of the broadest spectrums of any topical ophthalmic antibiotic, covering gram-positive, gram-negative, and anaerobic bacteria, making it a first-line choice for empirical treatment of bacterial conjunctivitis in many clinical settings.

MECHANISM OF ACTION

Chloramphenicol inhibits bacterial protein synthesis by binding reversibly to the 50S ribosomal subunit at the peptidyl transferase center. This prevents the formation of peptide bonds between amino acids, halting the elongation of the polypeptide chain. The result is bacteriostatic activity against a wide range of organisms. Chloramphenicol penetrates ocular tissues effectively, achieving therapeutic concentrations in the cornea, aqueous humor, and vitreous body.

DOSAGE AND ADMINISTRATION

     Instill 1 to 2 drops into the affected eye(s) every 2 to 6 hours depending on severity

     For mild infections: 1 drop every 6 hours

     For severe infections: 1 drop every 2 hours initially, reducing frequency as the infection improves

     Treatment duration: 5 to 7 days, or as directed by the ophthalmologist

     Wash hands before and after each application

     Avoid touching the dropper tip to the eye or any surface

SIDE EFFECTS

Common:

     Mild stinging or burning upon instillation

     Temporary blurred vision

     Mild eye irritation

Less common:

     Allergic conjunctivitis or eyelid dermatitis

     Bone marrow suppression (aplastic anemia) with prolonged topical use, though extremely rare

     Overgrowth of non-susceptible organisms with extended therapy

NOT RECOMMENDED IN

     Known hypersensitivity to chloramphenicol

     Patients with a personal or family history of aplastic anemia or bone marrow depression

     Prolonged use beyond 10 days without physician review

     Concurrent use with other bone marrow suppressant drugs

DRUG INTERACTIONS

Chloramphenicol may inhibit hepatic CYP enzymes and increase effects of warfarin, phenytoin, and sulfonylureas, though systemic absorption from eye drops is minimal. Avoid concurrent use with other topical antibiotics that may be antagonistic. Space other eye drop preparations at least 5 minutes apart.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Protect from light. Keep tightly closed. Discard 4 weeks after opening. Keep out of reach of children.
