INDICATION
Olinc 600mg Injection contains lincomycin, a lincosamide antibiotic indicated for the treatment of serious infections caused by susceptible gram-positive organisms, particularly staphylococci and streptococci. It is prescribed for serious respiratory tract infections, skin and soft tissue infections, bone and joint infections (osteomyelitis, septic arthritis), and septicemia where the patient cannot tolerate or has failed treatment with penicillins or other first-line antibiotics. Lincomycin is reserved for serious infections where less toxic alternatives are not suitable.
MECHANISM OF ACTION
Lincomycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit at the 23S rRNA site, blocking the formation of peptide bonds and preventing elongation of the polypeptide chain. This results in bacteriostatic activity at standard doses, though bactericidal concentrations can be achieved against highly susceptible organisms. Lincomycin is active against most aerobic gram-positive cocci including Staphylococcus aureus and Streptococcus pyogenes, as well as many anaerobic bacteria.
DOSAGE AND ADMINISTRATION
● Adults: 600mg intramuscularly every 12 to 24 hours depending on severity, or 600mg to 1000mg IV every 8 to 12 hours for severe infections
● Children over 1 month: 10mg/kg intramuscularly every 12 to 24 hours, or 10 to 20mg/kg/day IV in divided doses
● IV administration: dilute in 100ml of normal saline or 5% dextrose and infuse over at least 1 hour
● Intramuscular injections should be given deeply into a large muscle mass
● Must be administered by trained healthcare personnel
● Dose reduction is required in severe hepatic impairment
SIDE EFFECTS
Common:
● Diarrhea, which may be severe
● Nausea, vomiting, and abdominal cramps
● Pain and induration at the intramuscular injection site
● Phlebitis at the intravenous infusion site
Less common:
● Pseudomembranous colitis caused by Clostridium difficile overgrowth, which can be life-threatening
● Hypersensitivity reactions including rash, urticaria, and anaphylaxis
● Blood dyscrasias including neutropenia and thrombocytopenia
● Hepatotoxicity with elevated liver enzymes and jaundice
NOT RECOMMENDED IN
● Known hypersensitivity to lincomycin or clindamycin (cross-sensitivity exists)
● Patients with a history of antibiotic-associated colitis or pseudomembranous colitis
● Neonates due to the risk of fatal gasping syndrome from benzyl alcohol preservative
● Meningitis, as lincomycin does not adequately penetrate the blood-brain barrier
DRUG INTERACTIONS
Concurrent use with neuromuscular blocking agents (tubocurarine, pancuronium) may potentiate neuromuscular blockade. Kaolin-pectin antidiarrheals reduce lincomycin absorption when given orally. Erythromycin and lincomycin may antagonize each other due to overlapping ribosomal binding sites and should not be used concurrently. Concurrent use with other potentially nephrotoxic or hepatotoxic drugs requires monitoring.
