INDICATION
Oflocin 0.6% Ear Drop is a fluoroquinolone antibiotic otic solution indicated for the treatment of acute otitis externa (swimmer's ear), chronic suppurative otitis media with perforated tympanic membrane, and acute otitis media in patients with tympanostomy tubes. Ofloxacin otic is one of the few antibiotics safe for use in the middle ear, as it is non-ototoxic, unlike aminoglycoside ear preparations which carry a risk of hearing damage.
MECHANISM OF ACTION
Ofloxacin inhibits bacterial DNA gyrase (topoisomerase II), the enzyme responsible for the negative supercoiling of bacterial DNA required for replication, transcription, and repair. By blocking this enzyme, ofloxacin prevents bacterial DNA synthesis, resulting in bactericidal activity. It demonstrates broad-spectrum activity against common otic pathogens including Pseudomonas aeruginosa, Staphylococcus aureus, Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis.
DOSAGE AND ADMINISTRATION
● Otitis externa (adults and children 6 months and above): instill 5 drops (for children 1 to 12 years) or 10 drops (for adults and children 13 and above) into the affected ear once daily for 7 days
● Chronic suppurative otitis media (adults and children 12 and above): instill 10 drops into the affected ear twice daily for 14 days
● Warm the bottle in your hands for 1 to 2 minutes before instillation to reduce dizziness
● Lie on the side with the affected ear facing up; remain in this position for 5 minutes after instillation
● Pull the outer ear gently upward and backward (in adults) or downward and backward (in children) to straighten the ear canal
● Avoid inserting the dropper tip into the ear canal
SIDE EFFECTS
Common:
● Mild ear discomfort or itching after instillation
● Altered taste (dysgeusia) due to drainage through the Eustachian tube
● Mild local irritation
Less common:
● Dizziness or vertigo, especially if the drops are cold
● Ear pain or ringing in the ears (tinnitus)
● Allergic reactions including rash, swelling, or pruritus
● Superinfection with fungi with prolonged use
NOT RECOMMENDED IN
● Known hypersensitivity to ofloxacin or any fluoroquinolone
● Viral or fungal ear infections
● Use in ears without a confirmed perforated eardrum in otitis media cases, as the middle ear should only be treated through a perforation or tube
