INDICATION

O-Monix 4mg/5ml Oral Suspension is a selective serotonin 5-HT3 receptor antagonist antiemetic indicated for the prevention and treatment of nausea and vomiting associated with emetogenic cancer chemotherapy, radiotherapy, and surgery in pediatric and adult patients. The liquid formulation allows precise weight-based dosing for children and patients who cannot swallow tablets. Ondansetron is one of the most effective antiemetics available, providing reliable control of both acute and delayed nausea.

MECHANISM OF ACTION

Ondansetron selectively blocks serotonin 5-HT3 receptors located both peripherally on vagal nerve terminals in the gastrointestinal tract and centrally in the chemoreceptor trigger zone (CTZ) of the area postrema. During chemotherapy, radiation, or surgical trauma, damaged mucosal cells release serotonin, which stimulates 5-HT3 receptors and triggers the vomiting reflex. By blocking these receptors at both sites, ondansetron interrupts the emetic pathway and prevents both nausea and vomiting.

DOSAGE AND ADMINISTRATION

     Children 6 months to 11 years: 0.15mg/kg body weight up to 3 times daily, starting 30 minutes before chemotherapy

     Children 4 to 11 years (post-operative): 4mg (5ml) before surgery

     Adults and children over 12 years: 8mg (10ml) twice daily for chemotherapy-induced nausea

     For post-operative nausea: 16mg (20ml) given 1 hour before anesthesia

     Shake well before measuring each dose with the provided device

     May be taken with or without food

SIDE EFFECTS

Common:

     Headache, which is the most commonly reported adverse effect

     Constipation

     Drowsiness and fatigue

     Dizziness

Less common:

     QT prolongation, particularly in patients with pre-existing cardiac conditions or electrolyte imbalances

     Serotonin syndrome when combined with other serotonergic drugs

     Transient elevation of liver enzymes

     Allergic reactions including rash (rare)

NOT RECOMMENDED IN

     Known hypersensitivity to ondansetron or any 5-HT3 receptor antagonist

     Concurrent use with apomorphine due to risk of severe hypotension

     Patients with congenital long QT syndrome

     Severe hepatic impairment (maximum dose 8mg daily)

DRUG INTERACTIONS

Concurrent use with apomorphine is contraindicated due to profound hypotension. Serotonergic drugs (SSRIs, SNRIs, tramadol, fentanyl) may increase serotonin syndrome risk. CYP3A4 inducers (rifampicin, phenytoin, carbamazepine) may decrease ondansetron effectiveness. Ondansetron may reduce the analgesic effect of tramadol.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Protect from light. Do not freeze. Keep bottle tightly closed. Shake well before use. Keep out of reach of children.
